The cortical motor hand area is located in precentral gyrus and it is like inverted omega or horizontal epsilon in shape. This area of pre central gyrus lesions and especially small corcical infarcts can cause to isolated hand weakness. Hand weakness may occur in all fingers or it is sometimes restricted to radial-sided fingers or ulnar-sided fingers. At this case, we present a 70-year-old male with isolated hand weakness. He had uniform weakness in right hand's all fingers with smail cor tical infarcts on the hand knob area due to plaque of the ipsilateral internal carotid artery. However small cortical infarcts on the hand knob is rare but cases with isolated hand weakness should be checked for cortical infarcts.
