In the cases with chronic renal failure when GFR starts to decline the phosphate excretion from renal tubules decreases. This leads serum calcium decrease and serum parathormone increase and with time endorgan resistance occures together with parathyroid hyperplasia. This is defined as tertiary hyperparathyroidism. In this case we will evaluate the surgical treatment and postoperative following in a patient with tertiary hyperparathyroidism occured in chronic renal failure.
