In this study, we aimed to evaluate the rare cause of the segmental infarct, among the differential diagnosis of acute scrotum. A 32-year-old patient was admitted to our clinic with scrotal pain. Radiological examination revealed an avascular lesion in the left testis. Tumor markers were negative. We performed partial orchiectomy, and pathology results showed segmental infarction of the left testis. In the differential diagnosis of acute scrotum, clinical and radiological findings should be evaluated together because segmental infarction can also be one of the reasons. Thus, unnecessary orchiectomy may be avoided. In addition, clinical nonprogressive follow-up may also be considered.
