A case report of medial meduller syndrome caused by vertebral artery dissection

Male patient, aged 40 is applied to the hospital with the complaints of right hemifascial buming sensatian and right hemiparesia. There was no important risk,factors, no tobacco and aleohal consumption.

During neurologic examinatıon; right hemiparesia, right hemihypoesthesia and decrease of tonus in right upper and tower extremities was present. The special disorder was seen in cranial CT, but there was subacute ischemic approxiıruıtelly 2 cm. lesion in medulla oblangata in the right paramedian region in cranial MR. Right vertebral artery was hypoplasic and there was fenestratıon of basillery artery ın MR Angiography. At the digital substration angiography, we observed dissection in VI segmends of both vertebral arteries and in V2 segmends in right vertebral artery.

Recent studies show ed that, vertebral artery atherosc I erosis is one of the major caııses ofmedial medullary syndrome. Vertebral artery dissection ıs a for less comman cause of this disease, so we planned to present this case of MMS caused by vertebral artery dissection.
